Introduction

This form allows you to:

you have the right to appeal the animal activity licence rating if you do not agree that our assessment reflects the risk or compliance rating found at the time of the inspection
you have 21 calendar days from the date of issue of the licence to lodge an appeal
your appeal will be heard and we will contact you with our decision within 21 calendar days of receipt
if a site visit is required to determine an appeal, you will be liable for the cost of the visit unless it results in a higher star rating being awarded

you have the right to request an inspection to re-assess your current rating
you can make a request for an inspection any time after the statutory inspection
we'll charge £149 for this inspection. There is no limit on the number of requests you wish to make. Details of the payment will be provided on the acknowledgement email following your submission
you must provide details of the improvements including supporting evidence where appropriate
an inspection will be made within three months after receiving your request

The form will take between five and ten minutes to complete depending on the request. You will need to provide the following:

your contact details
your business details
to appeal your rating you will need to provide reasons why you do not agree with the rating given by the officer
to request an inspection you will need to provide details of the actions you have taken to comply with the issues raised in the inspection report
